You do not need to submit vaccine information or medical exemption forms until we have a firm start date and your child has been approved into the program,

Completing the form does not bind you to the program should you decide you no longer wish to enrol in the program once a start date is announced.

Spaces may be limited and completing the form does not guarantee enrolment.
